I have many friends who go school in Asia, and all of them said Bocconi probably is the best place for a semester exchange. Don't know about every detail but courses are decent, and plenty of things to do at Milano.

Bocconi is a top school with a recognizable brand name. Not sure how much pull a single semester abroad can have (I assume that's the case here), but it can't hurt. Yes, classes are tough and the environment is highly competitive as everyone is driven and aiming to recruit solely for London and Milan. Find a handful of people you can get close to and ride it out as a group. Life in Milan is actually really great, you get an amazing QoL, have plenty of great options nearby to get out of the city and the nightlife is really cool too.

Italian exams are graded out of 30 and you also have "30 e Lode" (meaning 30 with Merit) as the grade above that. This means you did extremely well and pretty much nailed the exam. A good grade is probably 28/30 or above which is not that easy to get and consider that you need 18/30 to pass. I would say grades follow a normal distribution between 18 and 30 with Merit so as an average grade, a small number of students get 28+. Also consider that in Italy most exams are done orally instead of written so you also have factors such as how much the professor likes you that come into play. Realistically, you can get good grades with the right amount of prep and work. You will likely have to work harder than in the US.
