MBA Application Review - 2.9 UG GPA

I will be very grateful for ANY advice on what I can do to have a shot at a T15 school in the US, or a top 5 business school in Europe. Don't worry I am very aware that for these schools I would probably fall in the bottom 10%, if that. Would like to go for my MBA in 2021 (will have 5 years of WE at that point, however, I can postpone it if that'll help). If there are specific schools I have a better chance at that would be great as well.

GMAT: 690 - first attempt. Sitting for it again in 2 months.

Undergrad from a reputable Canadian University, Business Major. Undergrad GPA: 2.9. (had a horrendous first year - no excuses here). During my UG, worked in a commission based sales role for a school supplies company during the year. Spent every summer interning in different cities in Asia and Africa (Accounting, Product Dev, and IB internships).

Worked in the US for a mid market RE PE fund for 2 years (AUM: $1 Bn), then a private RE lender (AUM: $1 Bn) for 2 years in Toronto and now in RE Finance for a boutique bank (AUM: $3 bn), also in Toronto. Promoted to manager in both my previous roles, and am anticipating a promotion in my current role before I submit my application. All roles have been business development and underwriting/valuation focused. To date I've been responsible for $275-$300 million in transactions across all 3 roles. Will be able to get strong LROs from previous employers.

On the side I run my a business that I started with a partner 3 years ago- doesn't have anything to do with RE. I also have a small advisory shop that I run (2 clients currently lol), where I provide RE financing advisory to developers. Both these side gigs will be put on hold/terminated when I do my MBA - the main goal for them was to subsidize my MBA tuition.

EC: Coach and Board Member for a local risk youth project - responsible for coaching/mentoring youth as well as fund raising. Playing soccer competitively.
